TEAM APPLICATION

THE 11 DAY POWERPLAY COMMUNITY SHIFT 2019

July 5th – July 15th, 2019 at HarborCenter

As a team captain, you are putting together a team of 10-20 hockey players. All players will be required to pay a $25 non-refundable registration fee and be asked to set a $500 minimum fundraising goal. However, if a player does not raise at least $200 by the time of their shift check-in, that player will be required to pay the balance of $200 to cover expenses such as such as jersey and ice time. For example, if a player raises $150, they will be required to pay $50 at check-in.

As you build your team, let your players know that this is a fundraiser and that all funds raised will support The 11 Day Power Play and its chosen beneficiaries, Roswell Park Comprehensive Cancer Center, Make-A-Wish Western New York and Camp Good Days.

The 11 Day Power Play organization will work directly with you, as a team captain, to make sure you have everything you need to succeed. We rely on captains to pass on newsletters and information, motivate and inspire players to fundraise and prepare for the four-hour hockey shift, etc.

Captains who take full ice (captain two teams playing each other or have a set opponent) will be considered for acceptance first.
Captains with one team will be considered for acceptance second, based on available and appropriate team matches.

Waivers (HarborCenter, 11 Day Power Play, Excelsior Orthopedics) and registration fee will be collected upon fundraising page signup.

2019 PRELIMINARY TIMELINE

January 11 – Teams Announced
January 18 – First Captain Newsletter, including team instructions, player invitation procedures, waiver & registration fee collection, fundraising guidelines, etc.
February 15 – Schedule Released
March 8 – Captains’ Info Night (mandatory attendance)
April 23 – Final Team Rosters Due (Jerseys will be ordered based on this roster. ALL changes after this will be subject to original sizes ordered and MUST be communicated to Sara Schumacher immediately)
May 3 – Power Player Jersey Deadline (Special jersey to be worn on the ice by all those who have raised at least $1,100 by EOB Friday, May 3)
May 31 – Team Sponsor Deadline
